Transcatheter closure of perimembranous ventricular septal defects (pmVSDs) is a well-established procedure. Recently, Amplatzer duct occluders (ADO) I and II have been reported to close large series of pmVSDs successfully (off-label use). ADOs are economical compared with the standard Amplatzer VSD occluders, a major consideration in developing countries with low-budget programs. We report closure of symptomatic, hemodynamically significant pmVSDs using the ADOI and ADOII devices. Although there are no set criteria for choosing between ADOI and ADOII, the former's price tag includes snare and long sheath. Thus, we aim to predetermine device usage based on transthoracic echocardiography (TTE) findings. Between March 2013 and November 2014, 30 patients had transcatheter closure of pmVSDs using the ADO devices. The median age was 4 years (range 1.1-13 years) and median weight was 15 kg (range 6.5-85 kg). ADOII could not be used in VSDs larger than 6 mm and/or with a large aneurysm. The median VSD size as assessed by echocardiography was 5.5 mm while the mean was 5.5 mm (range 3-12 mm); while by angiography it was 5 mm & the mean was 4.75 mm (range 3-9 mm). The median fluoroscopy time (FT) was 8 min (range 5-38 min). We inserted ADOI in 13 patients and ADOII in 17 patients (no significant difference between median age and weight in each group). VSD size was significantly larger and FT was longer in ADOI patients; the device type matched what was decided from TTE data in 84% of cases. Follow-up ranged from 2 to 24 months (median 12 months). The mean LVEDD z-score of the patients was 1.1 before VSD closure, while it was 0.63, 0.35, and 0.23 at the 1-, 3 months, and last follow-up, respectively. Complete closure rates immediately, at 24 h, and at last follow-up were 87, 90, and 94% respectively. No patient developed heart block or any other complication. ADOI and ADOII are equally safe and effective in pmVSD closure. ADOII use, although cheaper than ADOI, is limited to smaller VSDs. The choice between ADOI and ADOII can be decided by TTE prior to procedure which is convenient in low economic programs.

Background Although the Korean version of the Autism Diagnostic Observation Schedule-2 (K-ADOS‐2) is widely being used to diagnose autism spectrum disorder (ASD) in South Korea, no previous study has examined the validity and reliability of all modules of K-ADOS-2 across a wide age range, particularly older children, adolescents, and adults. Method Data from 2,158 participants were included (mean age?=?79.7?months; 73.6% male): 1473 participants with ASD and 685 participants without ASD (Toddler?Module, n ?=?289; Module 1, n ?=?642; Module 2 n ?=?574; Module 3 n ?=?411; Module 4, n ?=?242). Participants completed a battery of tests, including the K-ADOS or K-ADOS-2 and other existing diagnostic instruments. Sensitivity, specificity, area under the receiver operating characteristic (ROC) curve, positive predictive value (PPV), negative predictive value (NPV), Cohen’s kappa ( k ), and agreement with existing diagnostic instruments were computed. Cronbach’s α values were also calculated. Results All developmental cells of the K-ADOS-2 showed sufficient ranges of sensitivity 85.4–100.0%; specificity, 80.4–96.8%; area under the ROC curve, .90-.97; PPV, 77.8–99.3%; NPV, 80.6–100.0%; and k values, .83–.92. The kappa agreements of developmental cells with existing diagnostic instruments ranged from .20 to .90. Cronbach’s α values ranged from .82 to .91 across all developmental cells. Limitation The best-estimate clinical diagnoses made in this study were not independent of the K-ADOS-2 scores. Some modules did not include balanced numbers of participants in terms of gender and diagnostic status. Conclusion The K-ADOS-2 is a valid and reliable instrument in diagnosing ASD in South Korea. Future studies exploring the effectiveness of the K-ADOS-2 in capturing restricted, repetitive behaviors and differentiating ASD from other developmental disabilities are needed.

Con base en narrativas de una hija adoptada, que conocemos en las reuniones del grupo, profundizamos también en la discusión sobre el modo como, en una trayectoria personal singular, se enlazan cuestiones identitarias y relacionales tanto cuanto concepciones más amplias sobre familia, parentesco, raza, género, clase y pertenencia. Estando de acuerdo con Strathern (1999) que las revelaciones de una nueva información de parentesco puede ser envuelta por cautelas y tensiones, una vez que trae impactos incrustados para la identidad personal y para los relacionamientos, reflexionamos sobre algunos de los embates entre la práctica de la adopción y el derecho a los orígenes.

Recipes,mode of preparation and methods of administrationwere noted and adequately reported in this work. Allplants mentioned by the respondents were collectedand carefully identified at the Forest HerbariumIbadan (FHI).Results: A total of 36 plant species belonging to 17families were reported to be useful. The mostrepresented family was Fabaceae with 5 differentspecies, closely followed by Anacardiaceae with 4species. Sixteen (16) recipes were reported, andprominent in these recipes include Alstonia boonei,Enantia chlorantha and Zingiber officinale.Conclusions: Respondents reiterated that plantmaterials used were collected from the wild andtraditional medicine is preferable in the treatment ofmalaria compared to Artemisinin-based combinationtherapies (ACT). We suggest furtherpharmacological studies to ascertain the potenciesof the reported plant species.

The revised ADOS algorithms, proposed byGotham et al. (J Autism Dev Disord 37:613-627, 2007),were investigated in an independent sample of 558 Dutchchildren (modules 1, 2 and 3). The revised algorithms leadto better balanced sensitivity and specificity for modules 2and 3, without losing efficiency of the classification.Including the restricted repetitive behaviour domain in thealgorithm contributes to a clinical ASD classification inmodules 2 and 3. For module 1, the results indicate lessimprovement, probably due to the low-functioning popu-lation. In most groups, the advantages of the revisedalgorithms are achieved without losing the strength of theoriginal algorithm.

Проанализирована динамика зрительных функций у 427 пациентов с аденомами гипофиза после эндоскопического эн-доназального транссфеноидального удаления опухоли. Исследование показало, что на послеоперационную динамику зрительных нарушений влияют не только особенности роста и размер опухоли, исходное состояние зрения пациента, но и степень радикальности проведенной операции, и возраст пациента. Выявлено, что внедрение методики эндоскопической эндоназальной транссфеноидальной аденомэктомии не приводит к ухудшению клинических результатов в сравнении с применявшимися ранее методиками микроскопического трансназального удаления без дополнительного использования эндоскопа и с использованием такового на отдельных этапах операции.

In this article, Spanish sports policies and their influence on Olympic performance are analyzed, focusing on the program of Support for Olympic Sportsmen (ADO Plan, by the acronym in Spanish). It is described below how the ADO scholarships have been distributed and what has been the evolution of the Spanish Olympic participation under this program that supports the technification and professionalization of the national elite sport. The study continues with the analysis of the performance of Spanish athletes in the different editions of the Games within the development of this program, with special attention to the 2005-2016 period, emphasizing the relationship between the global economic contribution of the ADO Plan through scholarships and the results obtained. In conclusion, it can be seen that the ADO Plan has been one of the key factors in sports transformation in Spain and international successes since Barcelona 1992.
