Engineering societies can play a produtive role in the standardization of analysis and test approaches which have the purposes of failure analysis through diagnosis, and failure avoidance through prognosis and corrective action. Resulting standards and certification programs can lead to substantial cost savings, avoidance of federal regulation violations, and can help justify ISO 9001 compliance. The efforts in this area of one major society, the Society of Tribologists and Lubrication Engineers (STLE), are described, along with some history and future plans. In cooperation with the Vibration Institute, and specifically the MFPT society, STLE is currently involved in the initiation or improvement of various ASTM, ANSI, and ISO machinery vibration and oil analysis specifications, as well as the development of an ISO "Certification Program Development" standard. STLE has five years of successful experience with its Certified Lubrication Specialist certification program. Encouraged by this success, STLE has initiated an Oil Monitoring Analysis ("OMA") testing program, which will shortly evolve into a full certification effort. Unbiased state-of-the-art educational modules are available to support these certification programs, and are undergoing continuous improvement. Future plans include a machine tool/ cutting fluid selection, diagnostics, and reclamation certification program, and a certification program concerning the appropriate application of mechanical seals and diagnosis of their problems.

The violent environment to be considered is severe transient motion; in the environmental test community they are called mechanical shocks. A series of tests was conducted to evaluate several transient shock motion analysis methods to determine which is the best indicator of shock severity for facilities equipment. Six squirrel cage blowers were exposed to six different violent shock loadings, each of which could be progressively increased until the blower failed. Five of the test did indeed fail blowers. The transient acceleration time histories were analyzed by several methods. These included Fourier transform magnitude, pseudovelocity and acceleration shock spectra, and peak g levels of the time histories. The analysis that does the best job is the damped shock spectrum plotted with pseudovelocity as the ordinate on tripartite paper. It shows the five failure causing environments similar, and the sixth environment well below those five. The paper describes the tests, illustrates the different ways in which the blowers failed, discusses the data analysis procedures, and presents the data analyses. The study certainly proves that peak g's do not indicate the failure causing capability of a violent environment, a misconception deeply rooted in most of the shock design literature.

The Army Research Laboratory -Weapons and Materials Research Directorate (ARL-WMRD) performed a failure analysis of a Shock Strut Mount from an Army attack helicopter, which is a component of the main landing gear assembly. The Right Hand (R/H) mount, which secures the R/H landing gear trailing arm to the fuselage failed in flight. It was the first of these mounts to fail during flight. Stress corrosion cracking, which initiates at the transition point between the base of the underside of the shaft and the mount base, has been attributed to numerous failures of this item in the past. This has been the result of cadmium wearing off and corroding away in service. The part under investigation failed after 1148 flight hours, and the gross weight of the aircraft at the time of failure was 12,040 pounds.

This paper looks at quality and reliability from a dynamic and correlated perspective. A reliability-based Bayesian approach is presented for predicting the computer system integration fallout (quality) due to hard disk drive (HDD) based on HDD producer's Final Quality Audit (FQA) results. This approach extended the understanding and quantification of the traditional rectifying inspection concept to a broader perspective: (1) Quality can be regarded as reliability in a (short) time interval of (t, t+Δt). (2) A functional quality inspection (conformance test) may be regarded as a "short-term life test" with duration of Δt. (3) 100% inspection (or retest) upon lot rejection may be regarded as a running-in or stress screening [5; 6] prior to customer system integration or field usage. (4) The quality level manifested at customer system integration may be quantified using conditional failure probability that is a function of its inspection test history. (5) For a continuous production process, Bayesian approach which combines the historical test information with current sampling observation using Beta-Binomial-Beta conjugate family to update the quality estimate can be applied effectively. To assure the prediction accuracy, the following needs to be done: (1) Conduct a correlation analysis between producer's quality audit test and customer system integration process. (2) Choose a proper history length as the prior for the Bayesian prediction. (3) Adjust and select a proper confidence level.
