Background: Hearing loss poses a significant public health challenge as a common chronic condition with many known side effects that are often worse when left untreated. Aural rehabilitation (AR) is an umbrella term that encompasses a range of interventions (e.g., informational counseling and perceptual training) designed to reduce deficits related to hearing loss that may stand alone or be used in combination as part of a holistic plan. Purpose: This evidence-based clinical practice guideline is intended to inform the implementation of person-centered AR to adults with hearing loss. Given the well-known benefits of sensory management, it was not included within the scope of this guideline. The recommendations aim to help clinicians, individuals with hearing loss, and other stakeholders make evidence-informed treatment decisions and improve clinical outcomes, as well as provide payers and policymakers with information detailing a comprehensive approach to AR.

Purpose: The purpose of the present meta-analysis is to explore the potential effects of objective verification of hearing aid amplification on tinnitus-related outcomes. Method: Twenty-seven studies reporting tinnitus outcomes pre and post hearing aid fitting were identified through a systematic literature search. From these studies, data from 1,400 participants were included in the present meta-analy-sis. Studies were divided into subgroups based on whether they had reported performing objective verification of the participants’ hearing aid amplification or not. Outcome measures were tinnitus distress and tinnitus loudness. Results: Meta-analyses of all included studies indicated verified amplification to result in significantly enhanced reduction of tinnitus loudness (p < .00001), while the enhanced reduction of tinnitus distress only approached statistical significance (p = .07). However, when excluding an outlier from the subgroup of studies using unverified amplification, individuals receiving verified amplification showed significantly greater reduction of tinnitus distress (p = .02). In addition, analyses of longitudinal effects revealed that the reductions of tinnitus distress decreased over time among individuals receiving unverified amplification but increased over time among individuals receiving verified amplification. Conclusions: The present meta-analysis indicates verified hearing aid amplification to be superior to unverified amplification in terms of reduction of tinnitus loudness and distress. The longitudinal increase of mitigation of tinnitus distress with verified amplification only may reflect improved neural reorganization and/or better adherence to hearing aid use, with verified compared to unverified ampli-fication. Due to the low cost of hearing aid verification compared to the high societal cost of tinnitus, objective verification of hearing aid amplification for tin-nitus patients is recommended. ? 2022 American Speech-Language-Hearing Association.

PURPOSE: To determine levels of families' satisfaction and anxiety associated with the early hearing detection and intervention (EHDI) process in Massachusetts, and to assess the relationship between a child's hearing status and levels of family satisfaction. METHOD: Surveys were mailed to families whose infants (a) passed their initial hearing screening (Group 1), (b) did not pass their initial screening but subsequently passed an outpatient rescreen or diagnostic evaluation (Group 2), or (c) were identified with permanent hearing loss (Group 3). Survey instruments measured families' satisfaction and anxiety associated with each stage of the EHDI process. RESULTS: Of the 4,138 families surveyed, 1,106 (27%) responded. Families reported satisfaction with screening services (Group 1 = 88%; Group 2 = 86%; Group 3 = 75%), and few reported anxiety prior to the hearing screening (Group 1= 4%; Group 2 = 15%; Group 3 = 19%). Among families requiring retesting services, 97% of Group 2 and 87% of Group 3 families were satisfied. Among Group 3 families, 94% reported satisfaction with the care their audiologist was providing, and 79% were satisfied with their early intervention services overall. CONCLUSIONS: Families reported satisfaction with the EHDI services they received in Massachusetts and expressed strong support for the universal newborn hearing screening initiative.

Purpose: There is growing evidence supporting the use of third-wave psychological therapies, such as mindfulness-based interventions (MBIs) and acceptance and commitment therapy (ACT), for people with long-term or chronic physical health conditions. We conducted a systematic review and meta-analysis to critically evaluate the effectiveness of third-wave interventions for improving hearing-related distress and psychological well-being in people with audiological problems. Method: We searched online bibliographic databases and assessed study quality. We conducted random-effects meta-analyses if at least two randomized controlled trials (RCTs) examined hearing-related distress, depression, anxiety, or quality of life in people with audiological problems. Findings of pre-post studies were summarized narratively. Results: We identified 15 studies: six RCTs and nine pre-post studies. The methodological quality of studies was mostly poor to moderate, and sample sizes were typically small (overall n = 750). Most studies focused on tinnitus (n = 12), MBIs (n = 8), and ACT (n = 6). Statistically significant improvements in hearing-related distress were found with ACT and MBIs versus controls and other treatments at post-intervention in people with tinnitus, while improvements in depression and anxiety were only found for ACT versus controls at post-intervention. However, gains were either not maintained or not examined at follow-up, and there was no evidence for improvements in quality of life. Conclusions: At present, there is insufficient evidence to recommend the use of third-wave interventions for improving hearing-related distress or psychological well-being in people with audiological problems. There is some evidence that ACT and MBIs may be useful in addressing hearing-related distress in people with tinnitus, but only in the short term. However, findings should be interpreted with caution given the small number of studies with generally small sample sizes and mostly poor-to-moderate methodological quality. More high-quality, adequately powered, double-blind RCTs, particularly in audiological problems other than tinnitus, are needed to draw firm conclusions and meaningful clinical recommendations.

A review about technical and perceptual factors in hearing aid technology, research and development is provided, covering current commercial solutions, underlying models of hearing loss for usage in hearing devices and emerging future technical solutions for hearing aid functionalities. A chain of techniques has provided incremental, but steady increases in user benefit, e.g. in the fields of hearing aid amplification, feedback suppression, dynamic compression, noise reduction and situation adaptation. The models describing the perceptual consequences of sensorineural hearing impairment describe the effects on the acoustical level, the neurosensory level and the cognitive level and provide the framework for compensatory (or even substitutional) functions of hearing aids in terms of the attenuation component, the distortion component and the neural component of the hearing loss. A major factor is the requirement of a strong individualisation of hearing aid solutions calling for an appropriate assessment of the different sensorineural components of a hearing loss, especially with respect to bilateral and binaural hearing aid solutions.
