Previous scholarly studies have discussed the use of Zen Buddhism within the person-centred approach of Carl Rogers, demonstrating the feasible influence of Buddhism over Rogers’s theories. The present research delves into the convergence and divergence of person-centred therapy and the Mahāyāna (one of the current mainstreams of Buddhism) philosophy explicated within the Vimalakīrti Nirde?a Sūtra (a significant Mahāyāna canon); in particular, the bodhisattva spirit and four immeasurables, including loving-kindness, compassion, empathetic joy, and equanimity. Findings indicate that Carl Rogers’s counselling principles and practices comply with the Buddhist teachings of phenomenal vision, actualising tendency, and innate eagerness to alleviate suffering. This discussion also analyses ideas that have seldom been examined in person-centred principles, such as bodhisattva altruism, mind purity in nature and impurity caused by bewilderment, compassion fatigue, causes of suffering, and counselling techniques. Mahāyāna wisdom potentially offers references to the Rogerian family of theories, which also sheds light on the use of Buddhist-influenced non-medical interventions.

Telepsychotherapy represents a promising solution to problems pertaining to specialized mental health services accessibility, including when delivering psychotherapy to people who do not have access to care due to the COVID-19 pandemic. The quality of the working alliance established in such a therapeutic context remains often questioned. Moreover, no study has comparatively examined the evolution of the alliance over telepsychotherapy and conventional, face-to-face, psychotherapy. This study assesses the impact of cognitive-behavioral therapy administered via telepsychotherapy or face-to-face on the quality of the working alliance. One hundred and 15 participants suffering from generalized anxiety disorder (GAD) took part in this randomized controlled trial, 50 of whom were assigned to telepsychotherapy in videoconference and 65 of whom were assigned to conventional psychotherapy. Each client and their psychotherapist completed the Working Alliance Inventory every 2 sessions. In the current sample, telepsychotherapy did not interfere with the establishment of the working alliance over the course of the treatment for GAD. On the contrary, clients showed a stronger working alliance in telepsychotherapy delivered in videoconference than in conventional psychotherapy. Clients seemed to be more comfortable with telepsychotherapy than psychotherapists. The clinical implications of these findings are discussed.

Several well-established psychotherapies have seen a proliferation of their derivatives. Among notable examples are truncated forms of psychoanalysis, such as psychoanalytic psychotherapy and brief psychodynamic therapy; a variety of cognitive behavioral therapies (CBT) including CBT-I (CBT for insomnia), CBT-D (CBT for depression), and CPT (a trauma-focused CBT). Despite its relatively young age EMDR has also produced multiple modifications like ego states EMDR, EMDR for addiction, and EMDR for somatic conditions. The majority of modifications are developed by integrating new interventions with the original protocols. However, there is no commonly accepted ad hoc methodology or standard for such integration. This article suggests a “nested hierarchy” as a methodology for psychotherapy integration, exploring its ramifications for practice and research.

Early detection and prevention of psychosis has become an international priority. Much of this work has focused on youth presenting with attenuated symptoms of psychosis—those at clinical high risk for psychosis (CHR)-given their elevated probability of developing the full disorder in subsequent years. Individuals at CHR may be prone to exacerbated psychological distress during the coronavirus 2019 (COVID-19) pandemic and its subsequent physical isolation measures, as a result of heightened stress sensitivity and comorbid mental health problems. Telepsychotherapy holds promise for reaching this population, especially during the current COVID-19 outbreak. However, there are limited evidence-based guidelines or interventions for use of telepsychotherapy with this population. In this article, we review common clinical issues for individuals at CHR and how they might be exacerbated by the COVID-19 pandemic. We then review best practices for treatment and adaptations for telepsychotherapy for individuals at CHR, and highlight real clinical issues that we are currently experiencing in a United States-based specialized CHR clinic as we conduct telepsychotherapy via videoconferencing. We conclude with questions for those in the field to contemplate, as well as potential challenges and benefits in using telepsychotherapy with individuals at CHR and their families.

Some abortions can result in prolonged, complex grief reactions where the effects endure in problematic ways.? Research findings suggest that such women suffer a turbulent spiral of often contradictory emotions and, if insufficiently acknowledged at the time of the abortion by self or other, this emotional turmoil can become pushed down and disenfranchised. This paper discusses the nature of this entrapped grief by reviewing the literature and reporting on some phenomenological case study research. A final section discusses integrative psychotherapy approaches to working in this area. Better understanding of the experience of traumatic abortion may enable therapists to be more present to this phenomenon, in particular the voicelessness some clients may experience, rendering them unable to process their feelings about the event and associated loss.

In their article, the authors present the findings of the study in which they conceptualized the method of psychotherapy with the Parent ego state. Their aim was to explore whether this method could be divided into individual, content-wise separate chronological phases which can be observed with the majority of clients. By using a modified method of content analysis of five psychotherapy transcripts and a video recording of a psychotherapy session, nine chronological phases were identified. In order to illustrate the individual phases, excerpts from the transcripts and the video recording of psychotherapy have been included. The article proposes under what conditions can this method be used, and presents some of its limitations.

This article presents a response to the paper “Relational Withdrawal, Attunement to Silence: Psychotherapy of the Schizoid Process” by Richard Erskine in which he described his treatment of a client who made use of schizoid processes and defenses to manage her experience. The current author responds by focusing on the self-protective defenses of schizoid compromise and withdrawal that individuals with a schizoid presentation employ. Their struggle with attachments is also examined, and various theoretical perspectives on developmental processes and the structure of the mind are discussed. The spectrum of schizoid processes is examined from the more extreme introverted schizoid personalities who exhibit severe withdrawn presentations to those who seek some form of attachment. The nature of the internal world and the difficulty of managing relationships is explored. Therapeutic action in response to the schizoid compromise and withdrawn individual is considered. The article includes a number of clinical descriptions and concludes by exploring the processes involved when the therapist themselves occupies a schizoid compromise position.

Author (in press) proposed a transtheoretic transdiagnostic psychotherapy (TTP) based on an Applied Psychological Science (APS) clinical orientation founded on a Bio?Psychology Network (BPN) explanatory system that provides sufficient theoretical unification to support psychotherapy integration. A goal of transactional analytic psychotherapy is to ‘strengthen the Adult’: to develop a thinking, analytical part of Self that can manage overwhelming, problematic emotions. However, this understanding misses the layered complexity of what the Adult ego state offers and can also promote an unhelpful dissociation in the client which counters the essence and intention of Integrative Psychotherapy. In this article, I offer a way of conceptualising work with the Adult ego state as nurturing a client’s Integrating Adult – the cohesive, ‘grown-up’ part of our thinking-feeling-experiencing Self in the here-and-now, free of contaminations from old Parent and Child scripts. The article opens with an explicitly theoretical exploration of the TA concept of ‘Adult.’ In the second, applied section, I explore how we might work with the Adult ego state in our Integrative Psychotherapy work via three processes: decontaminating the Adult from Parent prejudices; deconfusing the Child; and nourishing the integrating Adult. The final section offers a case study illustration focused on nurturing a client’s Integrating Adult.

This article describes a qualitative study of helpful aspects of the therapeutic relationship in Integrative Psychotherapy. Participants of the study were sixteen clients who were in the process of Integrative Psychotherapy for at least a year. Participants were interviewed with the adapted version of the Change Interview (Elliott, 1999), which involves a semi-structured empathic exploration of the client's experience in therapy. The analysis of the clients’ experience of Integrative Psychotherapy revealed six categories of helpful aspects of therapeutic relationship the therapist’s empathic attunement, the therapist’s acceptance, the match between the client and the therapist, feelings of trust and safety, feeling of connection, and experience of a new relational experience. Based on results of the research, we developed a model of the healing relationship in integrative psychotherapy. This model describes the interrelatedness of these six helpful aspects of the therapeutic relationship. The categories of empathic attunement and acceptance proved to be the most important categories relating to the therapist’s contribution to the healing therapeutic relationship. Clients described that the therapist’s empathic attunement and acceptance influenced the development of safety and trust, feelings of connection and promotion of new relational experiences. The results of this study are discussed in relation to the theories of Integrative Psychotherapy and research regarding the therapeutic relationship in psychotherapy.
