These organizational... 展开 Recently, we have had a run of disasters, such as JCO criticality accident in Tokai-mura, that could not be explained by a single human error, but originated at issues of management and organizational factors. These organizational accidents look like different in their fields and scenarios, however; potential problems in upstream seems to have much similarity. That is, the potential path way has been formed due to degraded organizational factors and problems located in local workplace, so this path way have been attacking barriers continuously. Eventually, an unexpected event triggers the hazard released into a disaster. Here, authors describe common factors found in several organizational factors, and propose how to create the desirable safety culture in an organization. 收起
