It includes a wide spectrum of liver diseasesranging from simple fatty liver to non-alcoholic steatohepatitis（NASH）, which may progress to fibrosis andmore severe liver complications such as cirrhosis,hepatocellular carcinoma and liver mortality. NAFLDis strongly associated with obesity, insulin resistance,hypertension, and dyslipidaemia, and is now regarded asthe liver manifestation of the metabolic syndrome. Theincreased mortality of patients with NAFLD is primarily aresult of cardiovascular disease and, to a lesser extent,to liver related diseases. Increased oxidative stresshas been reported in both patients with NAFLD andpatient with cardiovascular risk factors. Thus, oxidativestress represents a shared pathophysiological disorderbetween the two conditions. Several therapeuticstrategies targeting oxidative stress reduction in patientswith NAFLD have been proposed, with conflictingresults. In particular, vitamin E supplementation hasbeen suggested for the treatment of non-diabetic,non-cirrhotic adults with active NASH, although thisrecommendation is based only on the results of asingle randomized controlled trial. Other antioxidanttreatments suggested are resveratrol, silybin, L-carnitineand pentoxiphylline. No trial so far, has evaluatedthe cardiovascular effects of antioxidant treatment inpatients with NAFLD. New, large-scale studies includingas end-point also the assessment of the atherosclerosismarkers are needed. 收起
